Before filling out this form, say/do the following to/for the customer:

1) We may already have all your information on file, what's the spelling of your first and last name so I can check to see if we have all your updated contact information? 

2) Look up the customers profile in HubSpot. If you find a matching profile, verify the customers information in his/her HubSpot profile starting with:

a) verify his or her bike information by saying: To confirm I have the right profile, you have a [insert Harley year] [insert Harley model] and you live in [insert customers City, State] - is that all correct?

If yes, pre-fill the form below for the customer.
If no (and they don't have a profile with us), complete the information below one field at a time.
